- Registration & Drop/Add Form - This should only be used if registration cannot be done in Self Service and/or if signatures of instructors and/or department chairs are required.
- Declaration of Major or Minor or Certificate - Students must obtain the name of their new advisor and signature both from the major’s department chair before submitting to the Registrar’s Office.
- Academic Standards & Policies (AS&P) Petition - Students should submit this form to request that the AS&P committee review the student’s request(s) for an exception to a policy and/or deadline. The student must obtain the required documentation and signatures before submitting the form and all documentation to the Registrar’s Office.
- Transfer Course Request Form - Students who wish to take coursework at another institution should complete this form before registering for courses elsewhere.
- Incomplete Request Form - This form must be submitted to the Registrar’s Office by the last day of classes with all required signatures.
- Withdrawal from Class - This form must be submitted to the Registrar’s Office by the course withdrawal deadline stated in the academic calendar with all required signatures.
- Withdrawal from the College - Students who wish to no longer be an active student at Hood College or will not be taking courses for more than two consecutive fall/spring semesters should complete this form.
- Leave of Absence Petition - Students who cannot enroll in fall and/or spring semesters should complete the Leave of Absence petition to remain an active student at Hood College. Students may take two consecutive Leaves but must enroll after that or be withdrawn. Failure to enroll or complete the Leave of Absence form for fall and/or spring semesters will result in withdrawal from the College.
- Undergraduate Application for Graduation - This form is required of all students completing bachelor's degrees. The names of all graduation candidates are presented to the faculty and board of trustees for approval several months before for each graduation date (January, May and September). Adherence to submission deadlines is essential.
- 4PLUS Dual Degree Request Form - Undergraduate students who are interested in pursuing one of the 4PLUS programs must complete this request form, which will be reviewed by the Registrar's Office, the student’s academic advisor, and the graduate program director for possible approval. - 4PLUS Undergraduate Dual Degree Enrollment Form – Undergraduate students approved to pursue a 4PLUS program must complete this form to register up to 9 credits of graduate coursework.
- Undergraduate Request for Graduate Coursework Registration - Undergraduate students who wish to take graduate level coursework (500 level) for undergraduate or graduate credit must complete this form, request an eligibility check by the Registrar's Office, and obtain approvals from their instructor and advisor.

- Enrollment/Degree Verification - Hood College has authorized the National Student Clearinghouse to provide degree and enrollment verification. National Student Clearinghouse may be contacted at: www.degreeverify.com or www.studentclearinghouse.org. You may also request the Registrar's office to complete an enrollment and/or degree verification by submitting this form to registrar@hood.edu.
- Honors Core Exemption Form - Students in the Honors Program may apply up to 9 credits towards the Methods of Inquiry section of the Core Curriculum for completing HON 102, HON 201 and/or HON 202. Exemptions can only be granted for completed coursework.

- Petition to Study Away or Abroad (Undergraduate) – Students who wish to study abroad or take a full courseload at another institution for a spring or fall semester should complete this form.
- Transfer Course Request Form (Undergraduate) – Students who wish to take coursework at another institution should complete this form before registering for courses elsewhere. Course equivalencies can be found at https://selfservice.hood.edu/SelfService/Planning/TransferCourse.
- Withdrawal from the College (Undergraduate) – Students who wish to no longer be an active student at Hood College or will not be taking courses for more than two consecutive fall/spring semesters should complete this form.
- Withdrawal from Class (Undergraduate) – This form must be submitted to the Registrar’s Office by the course withdrawal deadline stated in the academic calendar with all required signatures.

- Change of Undergraduate Major/Graduate Program/Minor/Program Advisor
- Change of Address - Address changes should be made through Self Service. The Registrar's Office will be notified electronically of the changes and will update Self Service accordingly.
- Name Change - Students must also submit a copy of an official document with the new legal name, which can be uploaded onto this form or emailed separately to registrar@hood.edu.
- FERPA Release of Information – This form allows students to grant permission for their educational records only to be shared with other individuals of the student’s choosing. This form must be initiated and completed by the student. The offices of Financial Aid and Accounting have different FERPA forms.
- Undergraduate Application for Graduation - This form is required of all students completing bachelor's degrees. - Undergraduate Change of Expected Graduation Date - Students who need to update their expected graduation date should complete this form.
- Anticipated Degree Conferral Letter request form - Students who complete all degree requirements and are waiting for their degrees to be conferred, can request a letter of anticipated degree conferral from the Registrar's office using this form. Some employers or educational institutions may require such a letter for reimbursement, promotion or admission. Students should allow at least a week for such a request to be processed.
- CPT Form - Curricular practical training is temporary employment authorization for F-1 visa non-immigrant foreign students in the U.S. while enrolled in a college-level degree program. Enrollment in internship coursework is required to meet CPT requirements.
- International Student Commencement Verification Form - International students who wish to verify their graduation and commencement participation for the purpose of inviting family members from home countries to the commencement event in May. Students can complete the top portion of the form and submit to the Registrar's Office for verification and signature; the form can be provided to family members in efforts of obtaining B-2 visitor visas.
- OPT Anticipated Graduation Form - Students on F1 visas who plan on applying for Optional Practical Training (OPT) after graduation must complete the top portion of this form, submit to the Registrar's Office for verification, then submit to the Primary Designated School Officer (PDSO) for final approval and processing.
- Readmission Request (Undergraduate) - Students who are accepted for readmission to Hood College enter under the catalog requirements active at the time of readmission, unless the student specifically requests on their readmission application to be readmitted under the catalog active at the time of their earlier attendance. The College may approve or deny this request based on the number and type of courses which the student has left to complete. If this request is denied, students will be expected to complete degree requirements in effect at the time of readmission to the institution.
- Undergraduate Non-Degree Application/Registration Form - This form is used for those who wish to enroll in undergraduate coursework as non-degree seeking. A transcript may be required if coursework has prerequisites, which can be emailed to registrar@hood.edu. Students will be emailed once the registration has been processed.
- Diploma or Certificate Reprint Request - Alumni who would like a reprint of their diploma and/or certificate (graduate certificate only), can do so by completing this form and submitting the $35 fee. Reprint orders are submitted to the printing office the 1st and 15th of every month and may take a few weeks to process and fulfill.
- Verification of Prior Enrollment - Former students who may need a verification of prior enrollment may complete this request form and submit it to the registrar's office. An official letter will be provided as requestor indicates on the request form.
